How to Get Referred

To participate in the Cardiovascular Prevention and Rehabilitation Program, you will need a referral from your doctor.

Ask your doctor for a referral to our program if you have any of the following conditions:

  • heart attack
  • heart surgery
  • angioplasty
  • heart failure
  • other heart conditions, such as angina or pacemaker
  • other vascular conditions, such as stroke or peripheral arterial disease
  • high risk of heart disease, such as family history of heart disease, high cholesterol, high blood pressure, or diabetes

When making a referral, your doctor will include:​​

  • a letter of referral indicating the reason for cardiac rehabilitation
  • clinical notes relevant to the referral
  • tests done, if relevant
